Paying for medical care in Shanghai: cards, Alipay, WeChat Pay

Medical fees at Shanghai hospitals are always paid directly to the hospital. Our coordination fee is separate; we never add a markup to medical fees and we do not accept payment on a hospital’s behalf.

Before you fly: set up mobile payment

QR-code payments dominate retail and healthcare payments in Shanghai. Both major platforms now support international bank cards, and setup is easier at home with a stable connection.

Alipay (international version)

Alipay offers an international version “tailored for overseas travellers to China, aiming to fulfil their mobile payment needs in China” (Alipay+, pay-in-the-chinese-mainland guide, accessed 2026-07). Overseas users can “download Alipay, choose the ‘International Version’ after signing up, and bind a bank card” (Alipay+, pay-in-the-chinese-mainland guide, accessed 2026-07).

Supported cards: “International bank cards refer to cards of major international card networks, including credit and debit cards, issued outside the Chinese mainland” (Alipay+, pay-in-the-chinese-mainland guide, accessed 2026-07). The Shanghai Municipal Government’s guide to the service names Visa, Mastercard, Diners Club International, and Discover as accepted networks (Shanghai.gov.cn, guide to linking international bank cards to Alipay, accessed 2026-07).

What international cards cannot do: International bank cards “do not support person-to-person transfers, sending or receiving red packets, wealth management services, insurance services, or other financial services” — they are for merchant payments only (Alipay+, pay-in-the-chinese-mainland guide, accessed 2026-07).

Transaction limits: Following April 2024 improvements to payment services for foreign nationals, Alipay raised the single-transaction limit to USD 5,000 and the annual limit to USD 50,000 (Beijing.gov.cn, Alipay Further Optimizes Payment Services for Foreign Nationals in China, accessed 2026-07).

WeChat Pay (Weixin Pay)

WeChat Pay allows international users to link their international bank cards directly to Weixin Pay accounts for use at merchants within China’s mainland (WeChat Help Center, Guide to Add and Use International Cards with Weixin Pay, accessed 2026-07).

Supported card networks: Visa, Mastercard, American Express, Discover Global Network, JCB, Diners Club, and UnionPay (Beijing.gov.cn, WeChat Pay Exempts 3% Transaction Fees for International Card Purchases Under CNY 200, accessed 2026-07).

Transaction limits: Single-transaction limit CNY 6,500; monthly cumulative limit CNY 50,000; annual cumulative limit CNY 65,000 (WeChat Help Center, Guide to Add and Use International Cards with Weixin Pay, accessed 2026-07).

Fees: Transactions under CNY 200 are permanently exempt from the standard 3% transaction fee; a 3% fee applies to transactions of CNY 200 and above. New users linking an international card for the first time enjoy a 60-day promotional period with the 3% fee waived on daily transactions under CNY 1,000 (maximum saving CNY 30 per transaction) (Beijing.gov.cn, WeChat Pay Exempts 3% Transaction Fees for International Card Purchases Under CNY 200, accessed 2026-07). Promotional terms are time-limited and subject to change — treat the new-user waiver as a possible bonus, not a guarantee.

Identity and phone requirements: You can use a passport for identity verification; a Chinese Mainland (+86) phone number is not required — an international number that can receive SMS is sufficient (WeChat Help Center, How to Set Up International Cards for Weixin Pay, accessed 2026-07). Requirements can change; confirm the current identity and phone rules directly in the WeChat (Weixin) Help Center before you travel.

What international cards cannot do: International cards linked to Weixin Pay “do not support certain features, such as red packets and money transfers” — they are for direct purchases only (WeChat Help Center, Guide to Add and Use International Cards with Weixin Pay, accessed 2026-07).

At the hospital

Payment typically happens at the hospital cashier desk or self-service kiosk. International departments at major hospitals are accustomed to serving overseas patients, but accepted payment methods vary by hospital and may differ from what is accepted in general outpatient or retail settings.

Per-hospital methods still need a live check. Before you travel, we confirm with the department or international unit whether Alipay, WeChat Pay (including international-card top-up where applicable), Visa/Mastercard, or cash is accepted at the cashier for your planned procedure — lists change, and we do not invent a hospital’s payment policy.

Come prepared with more than one payment option. Hospital cashiers can be busy; having your mobile payment app set up in advance avoids delays at the counter.

Cash and ATMs

UnionPay’s official guidance for foreign visitors states that “cash withdrawal using UnionPay cards is supported by all ATMs in the Chinese Mainland,” including locations at airports, railway stations, and ports of entry (UnionPay International, Explore Mainland China Your Way, accessed 2026-07).

For cards issued outside mainland China, the per-transaction ATM withdrawal limit is CNY 3,000, applied “in accordance with the requirements of China’s State Administration of Foreign Exchange”; some ATMs may apply additional restrictions (UnionPay International, Explore Mainland China Your Way, accessed 2026-07).

Whether non-UnionPay international cards (Visa, Mastercard) are accepted at ATMs in mainland China is not addressed on the UnionPay International pages reviewed for this guide. Check with your home bank before travelling about China ATM compatibility, and carry some cash as a backup regardless of your primary payment method.
